Our office is now accepting CareCredit, the health, wellness, and personal care credit card.
Contact our office to pay with this credit card if you are already a cardholder. If you would like to use CareCredit to pay your bills at our office, please use this link to apply.

Non-Participating Insurance Plans

If we do not participate with your insurance plan, you can still be seen by a provider in this office. Payment in full will be expected at time of service. If your plan provides an out-of-network option, you can provide payment according to the terms of your policy.

Payment Policies

We are dedicated to providing the best possible care for you, and we want you to completely understand our financial policies.

Billing and Insurance

All insurance co-payments and deductibles are due at the time of service and are collected at check-in. If we do not have a contracted arrangement ("out of network") with your insurance plan, we will gladly prepare and file your medical charges for you. This means your insurance carrier may send the payment directly to you. Therefore, payment of all charges are due at the time of service. We accept cash, checks, MasterCard, Visa, Discover and Care Credit as payment options. Your insurance company requires you to present your insurance card at each visit. If the insurance company that you designate is incorrect, you will be responsible for payment.

Online Bill Pay

For your convenience you can pay bills on line. This service allows you to pay your bills quickly and securely through
the patient portal.

If we are your primary care physician, make sure our name or phone number appears on your card. If your insurance company has not yet been informed that we are your primary care physician, you may be financially responsible for charges.

Not all insurances cover all services your provider feels are necessary. In the event your insurance plan determines a service to be "not covered" you will be responsible for the service. Please take time to review your policy before your appointment. Questions regarding your insurance coverage should be directed to your insurance carrier or your employer's human resources department. We will make every effort to make certain that the services we recommend are covered by your insurance policy.
